BY CLICKING "I AGREE," CHECKING A RELATED BOX, USING ANY OTHER ACCEPTANCE PROTOCOL PRESENTED THROUGH THE SERVICE OR OTHERWISE AFFIRMATIVELY ACCEPTING THIS CONSENT, YOU ACKNOWLEDGE THAT YOU HAVE READ, ACCEPTED AND AGREED TO BE BOUND BY THIS CONSENT. IF YOU DO NOT AGREE, DO NOT CREATE AN ACCOUNT OR USE THE SERVICE. YOU GRANT AGENCY AUTHORITY TO ANY PARTY WHO INDICATES ACCEPTANCE ON YOUR BEHALF. IF YOU ARE EXPERIENCING A LIFE-THREATENING SITUATION SUCH AS CONTEMPLATING SUICIDE, CALL 911 OR THE 988 SUICIDE & CRISIS LIFELINE AT 988.
Purpose
This consent ("Consent") gives you information about telehealth and obtains your informed consent to the use of telehealth in the delivery of healthcare and/or mental health services to you by physicians, physician assistants, nurse practitioners and/or mental health professionals ("Providers") using the online platforms operated by LRX Local, LLC ("Service"). "You" means the person using the Service or, for a minor between 13 and 18 (or a higher age of majority under state law), both the consenting parent or legal guardian and the minor.
Use of telehealth
Telehealth is the delivery of healthcare and/or mental health services using electronic communications between a provider and a patient who are not in the same place. Through LRX Life, telehealth may be used for diagnosis, treatment, follow-up and patient education, and may include electronic transmission of medical records; photos, health information or other data; real-time audio and video; secure messaging and email; review of medical device data, sound and video files; and remote monitoring.
Alternative methods of care, such as in-person services, may be available, and you may choose them at any time. We encourage you to discuss all treatment options with your Provider.
Anticipated benefits
Telehealth can make it easier to access care at convenient times, from the privacy of your chosen location, without travel, and with ongoing access to your care team through secure messaging. For mental health services, participation may help with stress and emotional regulation and support more consistent access to care.
Potential risks
The quality, accuracy or effectiveness of services may be affected by the limits of virtual care. Technology may experience bugs, errors or outages. Providers cannot perform physical examinations or certain tests, which may limit their ability to diagnose or treat some conditions, and they may refer you to in-person or emergency care.
Delays may occur due to provider unavailability or technical problems. Despite security measures, electronic systems could be compromised, and electronic records and email may be more vulnerable to unauthorized access than paper records. Treatment options may be limited by regulations in some jurisdictions, particularly for prescriptions and controlled substances. Without complete medical records, there is a risk of adverse drug interactions, allergic reactions or other errors in medical judgment.
Telehealth relies heavily on the information you provide. Its accuracy and completeness significantly affect the quality of care. For mental health services, you may temporarily feel increased discomfort as therapy progresses.
Life-threatening and other emergencies; follow-up care
If you are experiencing a life-threatening situation such as contemplating suicide, call 911 or the 988 Suicide and Crisis Lifeline at 988. If the situation is an emergency, call 911.
Telehealth is not appropriate in some situations. If you need immediate or urgent care, go to an emergency room or other provider equipped for urgent care. Providers may not respond promptly to messages sent through the Service. For non-urgent matters, use the secure messaging in the Service.
Data privacy and protection
The Service uses network and software security protocols to protect your information. Information that identifies you or contains protected health information will not be disclosed to third parties without your consent, except as authorized by law for consultation, treatment, payment/billing, certain administrative purposes, mandatory reporting (e.g., danger to self or others; abuse of a child, elder or vulnerable adult), or as described in your Provider's Notice of Privacy Practices. The Service may include email communications containing your protected health information. LRX Life cannot guarantee the security or privacy of services you use to receive communications, such as your email provider.
Laboratory products and services
Some services may require an at-home diagnostic test provided by third-party laboratories. Neither LRX Local, LLC nor your Provider can guarantee the accuracy or reliability of these tests, which may produce false negative, false positive or inconclusive results, and errors in sample collection may affect diagnosis or treatment. Your Provider will help you understand results and may recommend additional testing or in-person evaluation.
